The Clinical Pharmacist is responsible for providing prescribed medications, drugs and other
pharmaceuticals as needed for adequate patient care, according to professional standards and
practices. In addition, the Clinical Pharmacist is responsible for providing patients and providers with
therapeutic assessments and selected disease management, working toward improving patient and
practice outcomes.

Responsibilities:
  • Performs pharmacy clinical functions conforming to California Board of Pharmacy (BOP) rules, regulations, and guidance.
  • Provides clinical pharmacy services, including but not limited therapeutic substitutions while adhering to the best clinical practices
  • Provides all clinical pharmacy services to TrueCare providers and patients in a legal and ethical manner following the best and most current clinical practices.
  • Acts as a clinical pharmacist for both San Marcos and Loma Alta Pharmacy and Consultant Pharmacist for other TrueCare health center sites.
  • Routinely communicates with physicians, nurses and patients regarding drug therapy.
  • Reviews and evaluates individual drug orders for appropriateness, safety, and cost.
  • Fills prescriptions in a safe, accurate, and timely manner.
  • Consults patients on their prescriptions.
  • Advises the Pharmaceutical and Therapeutics Committee, as requested.
  • Stays abreast with the TrueCare dispensing drug formulary and clinical knowledge.
  • Develops and updates clinical pharmacy policies and procedures annually in collaboration with the Pharmacy Management Team.
  • Contributes to the success of TrueCare by participating in quality improvement activities and meets productivity goal.

Required Qualifications:

  • Bachelor’s degree in pharmacy or Doctor of Pharmacy Degree (PharmD) from an accredited College of Pharmacy.
  • Two (2) years working as a pharmacist.
  • Computer proficiency with Practice Management and Electronic Health Record systems, and the Microsoft Office suite, including Outlook, Word and Excel.
Desired Qualifications:
  • Clinical pharmacy practice (inpatient or ambulatory) experience.
  • Bilingual in English and Spanish.
  • Knowledge of community health centers, public assistance programs, state and federal regulations, and managed care systems.

The pay range for this role is $55 - $82.50 on an hourly basis.
