Job Description

Pride Health is hiring a Case Manager RN (Home Health) to support our client’s medical facility in Barre, VT. This is a 20-week assignment (with the Possibility of extension or conversion) and a great opportunity to start working with a top-tier healthcare organization!

Location: Barre, VT 056441

Schedule: 36 guaranteed hours/week | (Day Shift – Monday–Friday, Weekend Rotation)

Contract Length: 20 Weeks.

Overview:

The Home Health Registered Nurse (RN) provides skilled nursing care and comprehensive case management to patients in their homes. The RN coordinates all aspects of patient care, ensuring quality outcomes through adherence to evidence-based practice and agency standards.

Key Responsibilities:

  • Conduct comprehensive patient assessments and complete OASIS documentation accurately and timely.
  • Develop, implement, and evaluate individualized care plans in collaboration with the patient, family, and interdisciplinary team.
  • Deliver skilled nursing services, monitor patient progress, and adjust care plans as needed.
  • Provide patient and family education on disease management, medications, and self-care.
  • Communicate effectively with physicians, case managers, and other healthcare professionals to coordinate care.
  • Participate in weekend rotation as scheduled.

Required Qualifications:

Licensure: Active Registered Nurse (RN) license in Vermont (or compact state eligibility).

Experience:

  • Minimum 1–2 years of recent home health nursing experience.
  • OASIS and case management experience required.
  • Prior travel nursing experience strongly preferred.
  • Skills: Strong assessment, documentation, and time-management abilities; proficient in electronic medical records.
